Please complete the registration form below to register your child for the 2021 First Holy Communion programme.  You will also need to make a £25 payment via the donate button which appears once the form is completed.

A copy of your child’s Baptism Certificate is also required. Please bring this with you to the first Parents Meeting which will be held in the church on the 16th January 2021 at 3.00pm all parents/guardians are required to attend this meeting.

In light of the current pandemic situation we are unsure about dates of sessions and the exact date the children will make their First Holy Communion.  We hope to have all the relevant information available for you at the January meeting, so please make every effort to attend.

Spaces are limited and will be allocated on a first come first served basis.  The deadline for application is 30th November 2020.

Please be aware that there will be a total of 7 Saturday sessions of which the children are expected to attend, failure to do so could result in the candidate not being able to receive their First Holy Communion this year.

Transport to/from the sessions is the responsibility of the parents/carers.


Please make us aware of any medical condition or regular medication. In the event of your child/ young person becoming ill or needing medical help we will get in touch with parents/ guardians immediately and / or call an ambulance, as appropriate.

In the event of an illness or accident every effort will be made by the event leader or their assistants to contact me. If for whatever reason this is not possible I agree to my son/daughter receiving medication as instructed and any emergency dental, medical or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by the medical authorities present.

Holy Rood Church recognises the need to ensure the welfare and safety of all children and young people. In accordance with our safeguarding policy we will not permit photographs, video or other images of children and young people to be taken without the consent of the parents/carers and children. Holy Rood Church will follow the guidance for the use of photographs, a copy of which is available from the safeguarding representative. Holy Rood church will take all steps to ensure these images are used solely for the purposes they are intended. If you become aware that these images are being used inappropriately you should inform Canon John Cunningham and the safeguarding representative immediately.
I consent to the Holy Rood Parish photographing or videoing my child/candidate and I understand that these images will be displayed in the following circumstances: Holy Rood Parish website or other documents publicising the church/activities over the coming year and I hereby agree to this.
